 Proper formatting of dialogue

Couple of questions pertaining to dialogue:

1) when dialogue occurs in the middle of a sentence, is the first letter of the dialogue capitalized, as in this example: Donovan was running ahead, and said "Hello everyone," as he passed the crowd. - Come to think of it, is the comma even needed after everyone???

2) If a character is thinking or intending to say the word, but doesn't actually vocalize it, does it still get quotes? Example: In his mind he asked "Why," but didn't have the courage to vocalize the question.
